Adult Day Health Care provided by Valley Health Services (VHS) is the ideal solution for adults who need daily support to maintain their wellbeing, but wish to remain independent in their own homes and communities rather than in facility-based care. We provide daily nursing, healthcare, social services, and more to area seniors. Join us Monday through Friday at our dedicated space located in Little Falls Hospital.

Together with you and your practitioner, our staff of trained nurses and nursing assistants will create a personalized care plan to help you meet your wellness goals during your time with us.

Our team will work with you to care for your mind and spirit as well as your body, including:
- Group outings, including shopping trips, movie theaters, favorite restaurants, and traveling to local events
- Fun and creative leisure activities
- Mental health / psychological counseling arrangements
- Pet therapy
- Assistance with community services
- On-site coordination with community, social, and healthcare agencies

Referrals:
Referrals are welcome from practitioners, family members, or community agencies. Once we receive a referral, program staff will schedule an on-site or home visit to assess how to best serve your needs.

Billing & Payment:
Charges for care and any additional services may be billed to the individual or to the proper social service agency if the individual is Medicaid-covered.
While the Adult Day Health Care program is located at Little Falls Hospital, it is run by the expert caregivers of Valley Health Services. We also offer a variety of services at our short-term rehabilitation and long-term care facility at 690 W. German Street in Herkimer, New York.
Valley Health Services delivers excellent inpatient and outpatient care in many areas including physical, occupational, and speech therapies, cardiac rehabilitation, and skilled nursing for sub-acute, palliative, and long-term care needs.
