Bassett Healthcare Network Practices Sustain Recognition as Patient-Centered Medical Homes

Cooperstown, NY – Through a rigorous process conducted by the National Committee for Quality Assurance (NCQA), 33 Bassett Healthcare Network practices have sustained their Patient-Centered Medical Home (NYS PCMH) recognition through the annual reporting program.
For patients, this translates to comprehensive, coordinated, accessible, quality care focused on the whole person.
The NYS Patient-Centered Medical Home (PCMH) model is designed to address all patient care needs, delivering high-quality, cost-effective primary care. In PCMHs, a primary care doctor leads a team that partners closely with a patient throughout life. Using a patient-centered, culturally appropriate, and team-based approach, the PCMH model coordinates patient care across the health system and has been associated with effective chronic disease management, increased patient and provider satisfaction, cost savings, improved quality of care, and increased preventive care.
“Congratulations on this outstanding accomplishment to the practice leaders who guide PCMH principles every day, and to many others who supported this process and work tirelessly to focus on the whole patient and their spectrum of care, leading to improved outcomes,” said Deanna Charles, MHA, Senior Vice President, Chief Ambulatory & Transformation Officer of Bassett Healthcare Network.
“PCMH puts patients at the center of their care, by guiding them through preventive care needs, managing chronic and acute illnesses, connecting them with community resources, and providing easy access to their care teams by phone, online, and in-person visits,” said Wendy Kiuber, PCMH Program Manager at Bassett Healthcare Network.
The New York State Patient-Centered Medical Home (NYS PCMH) Recognition Program is an exclusive model developed in collaboration with the National Committee for Quality Assurance. It is based on NCQA’s core PCMH model which aligns with health plans and supports the state’s initiative to improve primary care and promote the Triple Aim: better health, lower costs, and better patient experience.
Dr. Amy Grace, Chief of Primary Care at Bassett Healthcare Network, said: “Sustaining Patient-Centered Medical Home recognition under NCQA’s New York State Program means empowering patients to be key members of their own care teams and fostering a care culture that provides team support and a continuum of resources for the whole person. PCMH is key to coordinating high-quality and effective care and improving health outcomes in our communities.”
The mission of the National Committee for Quality Assurance (NCQA) is to improve the quality of healthcare through programs such as their PCMH Recognition program. NCQA’s Patient-Centered Medical Home Recognition program is the most widely adopted PCMH evaluation program in the country. More than 10,000 practices (with 50,000+ clinicians) are recognized through NCQA. More than 95 organizations support NCQA Recognition.
