At the Susquehanna Family Practice, our primary care physicians and practitioners are committed to providing your family with quality care, close to home.
We are located in the FoxCare Center, and we house the nationally-recognized, rural-based Gender Wellness Center.

Transgender Health Services
Please call to establish care, or to schedule an appointment for the following transgender health services:
- Trans-affirming mental health counseling across the lifespan, including consultation letters for surgery
- Trans-affirming medical care across the lifespan, including care for transgender and gender diverse youth
- Trans-affirming surgical care within network, including referrals to plastic surgery (top surgery, breast augmentation, tracheal shave, body contouring, facial surgery), hysterectomy, and orchiectomy
- Referrals for gender-affirming surgery out of network
- Legal referrals for transgender and gender diverse patients (name change and other legal services)
- On-site and didactic training for healthcare professional on transgender health

A.O. Fox Memorial Hospital – Susquehanna Family Practice has received and sustained National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home Recognition. The New York State (NYS) Patient-Centered Medical Home program leverages health technology that supports high quality and patient-centered coordinated care. Sustaining NCQA Recognition through annual ongoing reporting demonstrates that the practice has invested in the tools, systems, and processes needed to deliver high quality and patient-centered coordinated care.
